Thank you for completing this form ahead of our first conversation. Take your time, somewhere you are alone and won't be disturbed. There are no right or wrong answers; what you share is a snapshot of this moment.
Your partner receives their own form. What you share here will not be shared with your partner without your consent. Everything is protected by my professional confidentiality.
How I work with secrets. In couples therapy I work with an open secrets policy. Alongside our joint sessions, I also have individual sessions with each of you. What is shared with me there, or in a message or form, stays separate. What you confide in me individually I will not share with your partner without your consent, and I will not raise it myself in a joint session. I can explore with you what that secret means for you, for your relationship and for the therapy, and support you if you decide to share something yourself. This applies to both of you, in the same way.
An intake is not a guarantee. The intake conversation is an introduction and an exploration. Only afterwards do we decide together whether couples therapy is the right form of support at this moment. Sometimes individual support, or another form of help, is more suitable first. Where there is domestic violence, or an increased risk of it, I do not offer joint couples therapy.
